Mosaic is looking for a Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our team!
In this role, you will provide health care to individuals with intellectual and/or developmental disabilities to achieve their best possible health by scheduling medical appointments, assessing condition, reviewing medications, completing any necessary medical forms and by updating medical records.

What You'll Do:
Collaborate with community healthcare service providers, nutrition services, and behavior specialists to provide individuals served with quality care.
Coordinate and oversee all healthcare appointments including appropriate follow-ups.
Educate and enable the Direct Support Team to administer medications and perform other tasks according to Mosaic policies and state licensing requirements.
Maintain and review medical records of individuals served to monitor pertinent concerns and provide documentation detailing the delegation of healthcare procedures and nursing tasks.
SCHEDULE: Days (M-F), 40 hours, No Weekends, No Oncall, rotating holidays schedule.

What YOU’LL Need:
Active state nursing license in good standing.
BSN or ASN degree
Minimum of one year of LPN or RN experience or if new LPN 3 years of previous CNA/CMA required. - NEW GRADUATES WELCOME!
Must be able to lift, push and pull at least 50 pounds.
Valid Driver's license
Certified in CPR and First Aid (can be obtained after hire)
