Are you a dedicated Speech Language Pathologist (SLP) ready to make a difference in the lives of older adults? Join our Home Health Rehab Team at Commonheart Home Health, in partnership with Symbria, and be part of a passionate, employee-owned team where your contributions truly matter!

✨ Position Highlights ✨

Location: Cover the West Bend, WI area with potential to travel up to a 30-mile radius (we can consider a smaller territory).
Flexible Schedule: Set your own hours—no fixed start or end times!
No Home Health Experience Required!
Mileage Reimbursement!
Clinical Fellows Welcome! We offer supervision and training.
We are open to:
- Part-Time: Regular weekly hours between 20-29.
- PRN Status: Maximum flexibility with no minimum weekly hours.

Responsibilities

Develop and implement personalized treatment plans to meet your patients' needs.
Creatively execute clinical and educational programs to enhance speech therapy outcomes.
Collaborate with an interdisciplinary team while meeting productivity standards and completing required documentation.
Help patients achieve meaningful results and be a trusted partner to the communities you serve.

Qualifications

Graduate of an accredited school for Speech Language Pathologists.
Hold a valid state license.
Clinical Fellows are encouraged to apply!
While Home Health or related experience is a plus, it’s not required.
