State of Franklin Healthcare Associates (SOFHA) is a 100% Certified Employee-Owned, physician-led multi-specialty primary care group situated in Johnson City, Tennessee. Established in 1998, SOFHA has been delivering advanced primary healthcare services to patients across all age groups in the region. SOFHA's comprehensive healthcare delivery encompasses primary and specialty care physician practices, along with outpatient facilities and homecare services. These services include an acute care clinic, diabetes clinic, echo-cardiograms, clinical laboratory services, physical therapy, women's physical therapy, a Sleep Center, Comprehensive Breast Center, and Imaging Center. 

In 2024, SOFHA became employee-owned when they implemented an Employee Stock Ownership Plan (ESOP).
